ANOTHER AERIAL DEVICE CLOSE CALL: Montreal Ladder Company Water Way Failure

In what reminds us of the recent LODD of Lawrence Park (PA) Asst Chief (and Erie career FF) Mike Crotty, this recent incident happened to Montreal (CAN) FD Ladder Company 423 – a 1994 NovaQuintech SkyFive (100 ft 5 section) . They were setting up for elevated master stream operations at the 5th alarm recently and somehow they managed to disconnect the sliding ladderpipe assembly from the 4th section and failed to get it properly secured to the 5th section…..so when they charged the system with water, the parts that may be seen in the photo ejected themselves from the ladder and landed between the apparatus and the fire building. Fortunately-no Firefighters were in the way. No one in MFD safety or maintenance was aware of the American NIOSH alert bulletin or the previous incidents….appears to be a language barrier.
