FF Falls From Roof at Hosue Fire

I fell from single story roof when axe hold gave way on a icy roof with a 12/12 pitch in a severe snow storm. Our crew responded Mutual aid to a commercial structure fire. Upon arrival our crew was assigned to vent the roof. While venting the roof the axe hold slipped causing myself and the rotary saw to fall to the ground, fortunately I swam towards the ladder and it helped to slow my fall. After the fire it was discovered that the IC was aware that there was no attic space and evidently we were venting what amounted to a joist space. This task should have been done by pulling ceiling from within the structure.

Lessons Learned:

When responding mutual aid to a less experienced department we need to be sure to evaluate their tactics when it comes to our safety.
