As you know, Lt. Vincent A. Perez and Firefighter Paramedic Anthony M. Valerio were tragically killed in the Line of Duty last June when they were caught in a flashover. The internal safety investigation report on the June 2, 2011 fire that killed Lt. Perez and FF/PM Valerio has been posted on our home page with the permission of the SFFD. Lot’s to learn from thanks to the SFFD. Again, no better way to honor the Fallen Firefighters than to learn from what went right and what went wrong.
