On Sunday 11/21, E45 was operating a 1990 Pierce Arrow reserve, that was changed into early Sunday morning due to a mechanical issue with their front line. While the engine was being driven after a response, the radiator suffered a failure at the neck that acts as the fill location. This resulted in hot coolant spraying the entire crew seating area, injuring both firefighters. See picture #3538.
Upon inspection of 1533, it was noted that the engine cover inspection door that covers the radiator cap was missing. The reason for this door missing is still being investigated.
Inspect for the following:
• That the soft engine cover is secured to the Velcro fastener at the forward edge of the metal engine cover;
• That the twist lock inspection door covering the radiator cap is in place and secures properly (see attached picture #3563);
• That there is no evidence of corrosion or leaks at or around the neck of the radiator (see attached picture #3564);
• That there is an overflow tube attached to the fill neck;
If any of the above discrepancies are noted, contact your local Pierce dealer.
