That report was published exactly 59 days after the fire department submitted a request for a NIOSH investigation.
NIOSH’s FFFIPP continues to accept requests for serious injury and line-of-duty death investigations through the NIOSH website. Firefighters and other interested parties can see what investigations FFFIPP is currently working on by visiting the ongoing reports web page.
THE NEW REPORT TEMPLATE-A MESSAGE FROM THE CREW AT NIOSH TO FIREFIGHTERS & OFFICERS
You will notice the above report looks different. NIOSH FFFIPP redesigned its template to make these reports easier to read and use in the fire service. This template is a shift from a more traditional, text-heavy report format to one that is more visual, action-oriented, and focused on helping readers quickly answer:
- What happened?
- What mattered most?
- What can prevent this from happening again?
Some differences you might notice include:
- a stronger first page with key points, a concise summary, and a clearly labeled contributing factor and prevention action table
- a more visual timeline
- new sections like Fire Department Actions to Echo and Consider This
For those who love the text-heavy reports, don’t worry. The goal was to keep the technical depth and rigor of investigations while making the prevention lessons easier to find, share, and apply. You’ll still find all the incident details you need, just later in the report.
This template revision has been going on for over a year because NIOSH wanted to get it right. Thank you to all those in the fire service community who provided multiple, detailed, and critical reviews that were all used to get to this final version. Please note that while we are currently transitioning to the use of this new template, some investigations will retain the legacy template when they are published over the next few months.
